Billing Policies:
• We
will be more than glad to file your claim with your insurance
carrier; however, co-payment or deductible
is
due at the time services are rendered.
•
Please call 24 hours before appointment time to cancel. There
will be a $25.00 fee for appointments not cancelled.
•
Please arrive early for appointment and notify staff if you
have any insurance changes.
•
Failure to notify the staff of insurance changes, may result
in charges being transferred to patient responsibility.
•
Please notify staff of any address and phone number changes,
so that we may update patient information files.
•
There is a $30.00 fee for returned checks.
•
We try to verify insurance coverage. However, it is always
a quote of benefits, not a guarantee of coverage. Anything
not covered by a patient’s insurance will be put to patient
responsibility for payment.
•
There will be a $25.00 fee for FMLA form completion.
